If he's lying about his age to further his career (promoters more likely to invite him as a huge talent) then he's obviously cheating in other ways. So yes it does matter, even if he hasn't entered junior championships.
And he did enter the World U20 championships last year where he finished 5th in the 800m, so if he is an age cheat, he got the unfair advantage of gaining the experience from that competition (and pushing a real junior out of a cherished place in the final).

It is super normal for phenoms to look older than their age. They are performing at such a high level because they matured earlier than their peers.
Alan Webb looked like he was 35 in high school. Nobody called him an age cheat. Kipchoge was called an age cheat for over a decade. Then he ran 2:01 at age 37.

Here’s a great vid showing African age cheats. Supposedly these guys are 16-17 — the beat is the Eritrean who forgot to rwmove his wedding band, never mind his creeping baldness. Nijel Amos is there - his own agent admitted he was over age (21 in this race). Yes, pillars of fair play.
