Will the NCAA retroactively go back and DQ her from all of her xc meets? Seems like that needs to happen. So someone else will get to be an All-American and Tulane will drop from 2nd to 4th in the conference xc standings.

They never do that. Khan never had his results DQ'd, I feel bad for the Auburn kid that was runner up in the 1500 that year at SECS. It is a bit ridiculous that they do not do that though, Louisville can lose a basketball title for recruiting violations, but an athlete never loses a title in the NCAA even after being popped for doping.

Mechanically, how does this work? Are athletes serving provisional suspensions just on the honor system?
Meet organisers usually need to notify about the athletes invited in their races, and therefore notified if an athlete is not allowed to compete. Also, even if an athlete is provisionally suspended but his ban is not publicly known, organisers and federations are usually notified.
