Sure, in theory you will not take the substance until the TUE is approved, but that seems very naive. You are right. WADA's Prohibited List only prohibits the use of stimulants "in competition." For substances prohibited "in competition," the testing period begins at 11:59 pm the day before the event, and runs through the sample collection period after event.
In other words, Molly only needs a TUE if she plans to take the ADHD drugs close enough to the competition that she would fail test the day of the event.

Depends on the circumstances. For example a doctor that's treating you for something serious isn't going to wait for a TUE to be approved. In this case the TUE would be approved retroactively.

If it were life threatening and a banned substance is given, the odds are the substance will be long gone before the athlete competes again. Also, I can see circumstances where USADA/WADA would be lenient: Some serious accident where you are unconscious and taken to the ER. Meds are administered to save your life.
Yes, agreed your example of life-threatening circumstances is different than a drug prescribed for life-enhancement (e.g., TRT, psychotic stimulants, etc).

They are common enough that WADA offer Physician Guidelines for Male Hypogonadism, explaining in detail the conditions under which a TUE would be considered.
So f-ing wrong--Epo is always banned, can't get an epo tue. Some substances can be used out of competition without a tue, in comp with a tue.
I hate to break it to you, but athletes can get a TUE for EPO. WADA even has a "TUE Physician Guideline" explaining the process for doctors treating athletes with kidney disease or transplants.
