Engine running?
In no particular order:
1) They are homeless and living the van life (but in a car).
2) They are involved in some elicit behavior that for some reason requires they be in that parking lot for a while (customers coming to them perhaps?)
Anything else is probably a one off.

I’ve been seeing this more and more the past year. Running through neighborhoods I’ll see at least a half a dozen cars sitting still and running with people in them just mindlessly staring at their phones. I run a 25 minute loop and it’s very common to see the same people in their cars on the second loop.
All kinds of reasons.
1. homelessness is the largest and already mentioned - here in California tons of people live in their cars. Everywhere I go, I see at least a few people in cars. With the way things are going at least 10% of americans will live in their cars in the next few years.
2. maybe they got out, worked out/whatev and are now back in their cars, they just spent less time than you
3. they needed someplace quiet to read or even study, maybe they're listening to a program or book
4. they're waiting on someone who they have no idea when they will arrive - a throughiker or someone doing a sport - if they win and move to the next round could take a while. They could have got to a place early and are waiting for a check in time at a hotel or airbnb.
5. they're inebriated and driving off wouldn't be wise
6. wherever they are going, traffic is so bad that it would be more pleasant to wait
In most these situations, it seems going to a coffee shop or a library (well not where I live, they're full of homeless and junkies) would be more pleasant but there is a global pandemic, so you cant blame people for avoiding situations like this.

I don't know but I'd be more suspicious of someone spending 2 hours in the woods or 2 hours in a gym! Sup with that?

This is something I noticed quite some time ago (6-7 years). If you look for it, you'd be astounded how many people sit around in cars in parking lots for long periods of time. It's confounding as to what they are doing, and why they feel they need to sit in a car in a public parking lot to do it.

No problem with that unless you're hogging the equipment hogging on the equipment. Are you the chubby boi that sits on the flat bench eating Poptarts and playing Pokemon at my YMCA?

Yea OK. Working out, cleaning up, out in 2 hrs, or hiking/running for couple hrs are "worse".

Well, in my case, my kid has 1.5 or 2 hr swim practices that (due to covid) no parents are allowed in for. So, many of the parents just sit there. This is at a multiuse, large fitness complex. I actually just drop off an leave but I do return so it might appear I am sitting there... and some parents do in fact, sit there.
I just got back from taking my girls to choir practice, dropped then off, sat in my car and read for an hour, than drove them home after practice.
It could be Instacart shoppers waiting to grab next gig.
I like to practice music in a parked car. since it's super repetative. that way i don't have to drive my neighbors and roommates crazy. this is an activity that sometimes cops will pull guns on you for. since helpful karen types report you are smoking crack etc and don't have to be accountable for their fantasies/lies.
Man it’s so sad that none of you have caught some road head. You’re not going to see the other person in that case.
It could someone on a work trip, finishing up some work, and haven't found a hotel to stay at yet.
I've seen lazy people let their dogs out in bad weather and not want to go out in the weather themselves. (Sometimes they run their dogs by driving on a remote or dead end dirt road too.)
Also, on cross country road trips, breaks during the day, or sleeping overnight in the back of the car set up for car camping.
