Long story short, I had pain develop in the head of the 5th metatarsal develop around a year ago, and it is not healing. Does anyone have an idea of what could be going on? I got a MRI a few months ago that said it could have been a stress reaction, so I was placed in a boot for 4 weeks. However, after 4 weeks, the area started to hurt again once I started to walk more. Did I reinjure the area or what?

You don't have a stress fracture, david45. If you have discomfort, walking will not make it worse. Grow up, toughen up, and follow Malmo's plan.
