putting aside the likelihood that this is an alan alda inspired troll (the link to imdb on the first page of this thread, it astounds me that everyone seems to be agreeing that this other woman is "crazy". nothing in that purported email seems crazy at all. no threats, implied or express.
just be an adult with her. tell her it was fun while it lasted but you can't enjoy it anymore because of the guilt, or whatever the (near) truth is. (i say near truth because insulting her would be a bad idea, for a number of reasons including that there is no reason to be unduly mean to her.)
imagine for a second it was the other woman who decided to stop the annual encounters. what would you want her to say to you? would you want her to freak out and assume you'd tell her husband? would you be more or less likely to tell her husband if she lied and told you her kid has cancer? it seems obvious to me that you'd want her to be honest with you. so it seems just as obvious that you should be honest with her.
Everyone is all for cheating and covering it up as long as they are not the ones being cheated on. You have terrible double standards, but that makes sense since no one likes the idea of taking personal responsibility for their poor behavoir. Nevermind, the lives that become affected because of those decisions.

this just isn't true. the OP didn't ask for advice about talking to his wife or children, so there was no reason to mention that. now that you have, i'll simply point out that "coming clean" years after the fact does little to help the person who has been cheated on. the person admitting it at that point is likely seeking forgiveness in a selfish way.
recognizing this does not equate to saying that the original cheating is blameless. dealing with the guilt internally will often lead to much less pain and suffering for the other spouse than "coming clean" would.

This sounds fine to me. He's a different person now than he was 12 years ago. He does not wish to hurt his family or himself. Unless you want to claim some moral obligation, OP seems like a good person.
