Below is my account of a train accident that happened in San Diego this past March. Be warned, it's pretty graphic
On Friday night I went for a run along the Del Mar Cliff’s. I ran south along the railway tracks and had been out for about 15mins when I noticed a train heading north. I was running along the high cliff section that rises above the tracks for several hundred meters so I had a good view of the oncoming train. It was moving very fast (70mph) and just as I was starting to come back down to track level I notice a dark headed guy in black board shorts walk out from under a tree towards the tracks, right as the train was passing.
The train horn blasted continuously and I felt the train slow and break as this figure disappeared behind the engine and carriages. I did not immediately believe what I had seen. So I kept running slowly running down the trail that led to the tracks (9th ave. Del Mar). After the train passed I was immediately in the ditch at track level and did not see anything I started to think everything was alright, but it wasn’t to be. I started hearing screams and shrieks from people standing on the cliffs looking down on the track.
I slowly walked up the gravel until I got to the tracks. I looked down on the other side and there was the same young man I had seen lying on the rocks next to the tracks. I hesitated and looked up to the cliffs at people with cell phones. I started shouting call 911. People had been trying, but no one was getting signal. I guessed that we were at 8th ave and shouted the address. I then turned back towards this kid lying on the ground. One of his friends had joined him and was leaning over him. He was yelling for help. I slowly walked up and squatted over his body. I saw three main injuries; his right arm was mangled from the elbow down. His right hand was missing from the wrist down. His right leg was missing from the top of the knee diagonally through to a mangled inner calf. Finally, he had a head injury to his right temple. It was about as wide as a finger and I could see right in.
I stood up and pulled my t-shirt off. I leant over and got the attention of the kid. I told him my name and asked him his. His answer was Zyke and he was from Rancho Penosqitos. His friend and I tried to keep him still and comfortable. He was clearly going into shock and was thrashing around quite a bit, especially his head. After he was calmer I took a towel his friend had and placed it under his injured arm. I checked for bleeding and then wrapped it tightly. I took my T-Shirt and wrapped it around his wounded leg, checking for bleeding. I tied it hard and then looked at his head wound. Someone throw us one of his shoes and we used it as a pillow to protect his head from the rocks.
It had now been around 5mins since he had been hit. Time was crawling along, but I could still not hear any sirens. A guy with a walkie talkie ran down the tracks from the north. He turned out to be the train conductor. A railway policeman was not far behind him. They were the first uniformed responders on the scene. Zykes friend and I now tried to keep the him conscious. His pulse had gotten a lot weaker. He was chocking and coughing up blood so we turned him on his left side and I used my finger to wipe out blood from his mouth. I was very worried about his head injury. His eyes had blood in both pupils. Then after about 8mins his eyes rolled back into his head and he passed out. His breathing slowed and I was about to start mouth to mouth when he woke up and cough up blood all over us. Was that a good sign???
I heard sirens now and within another 2-3 minutes the fire department was on the tracks. They worked hard on the guy to stabilize him. They photographed and collected the missing limbs inside the tracks and then sheriffs’ deputies and train accident investigators took our statements.
Later, as I became aware of how much blood was on my arms chest and face, I noticed how many people had lined up looking at what ever was going on. I sat down and rubbed dirt on my arms and chest to get it off. I asked the paramedics for something to wipe off with but they had used everything they had brought down.
I ran back to the health club, straight to the bath room and rinsed off.
I do not know the condition of the Zyke, I am amazed he didn’t die in front of me on Friday night.
PSS - I did find out that this person passed away about three weeks after the accident.

obviously the woman doesn't really have an excuse, you can just feel a train moving if its close by even if you had earphones on. She's lucky she wasn't directly infront of it and that she lived. below the knee isn't so bad either.
I've been around many trains, played on train tracks, raced trains, yet never have I found myself being hit by them. I cross two train tracks every day to go running. I wear an ipod occassionally, really there just isn't an excuse to be hit by a train unless you're tied up or mentally handicapped.
If you're in a vehicle, you get out of the vehicle. Its not rocket science. I read a story where a family became stuck on RR crossing, saw the train coming about 1 minute out, according to witnesses, yet instead of getting out the driver kept trying to get through the barrier even though it was futile. Finally about 30 seconds before it hits, he gets out and along with the woman passenger who is hysterical, attempts to unbuckle all of the children passengers who can't unbuckle themselves. None survive. 2 adults, 3 children aged 5-12 or whatever. Darwin awards? maybe...still sad though. But a perfect example of how stupid people act when they are in situations only stupidity would place them in. Oh the train is coming, I'm going to drive across the tracks anyways. Oh I see the train about to hit me and my car's stuck, lets stay in the car. unbe-f***in-lievable.
Two things:
If you *do* get out of a car that's stalled on the tracks, walk *toward* the train. (Well, no, don't walk on the tracks themselves.) When the train hits it, the car's gonna go flying, whole or in parts, mostly in the direction the train is going. You want to be going the opposite way. (Yes, I realize that this isn't rocket science. OTOH my brother-in-law *is* a rocket scientist, but he's no rocket scientist--if you get my drift--and this is advice that he would find helpful.)
Miss Deaf Texas was killed maybe a year ago, by a train, and I just found these stories that makes it a little easier to understand--apparently she was text messaging at the time. I rank this right up there with people who read magazines while they cross the street:

No, actually quite often you cannot. Particularly if you are moving.
how come I can feel the oh-OH-OH-OH vibrations even when I'm running.
The ground shakes, and the air carries the vibration. I feel it on my skin. Even when a train is barely moving, the friction on the tracks creates reverberations that I feel.
Maybe I'm just attuned, but from my perspective, getting hit by a train is pretty f***ing stupid.
Damn. I agree you could say this is a pretty stupid thing to have happen, but then who hasn't done some stupid things.
Even WITHOUT an Ipod when I run, I've at times been totally oblivious to friends driving beside me and honking, and generally to the world around me. I probably could have been smashed by a car a number of times, especially with trails that come out of woods right beside roads, etc. Whether a runner/jogger is doing 5:00 or 10:00 pace, when you're in it you're in it. It's a freak thing and a horrible thing, but I don't think it's that hard to imagine this happening to an intelligent person.

It might be you just have some really loud trains where you are. I am serious. Some commuter rail trains, mostly newer models, actually make a lot less rumbling. If you tend to zone out while running, it is really not that far out of scenario. Most times, yes you would noticed them but its not that hard to miss them. Especially if there are other sounds around.

People respond in unusual ways to any rumbling or shaking under their feet. I regularly run over a bridge which is steel and concrete. Despite the appearance that a car could drive over it, the bridge actually shakes and sways as you run across it. People walking on it as I run over it never fail to look straight down at their feet with a puzzled expression. They never look around on the off chance something big is going to run them over.
People today seem to have lost the ability to look out for themselves. Anyone who tries to bike or run along a path that runs past a beach or park area crowded with people will constantly run into people who stand dazed in the way of traffic.
Sad story. Hopefully she'll be able to make as good a recovery as possible.
A note about railroad tracks. I believe that they are privately owned by the railroad company. Anyone running along the tracks would be trespassing. A group of us in college got stopped by the cops for running along the tracks. The railroad sent a nice little letter to our coach explaining the trespassing.
She was in shock, yes. Many patients with severed habds, legs, etc. feel a burning sensation, and do dnot realized what really happened.
Away from the medical side, she deserves sympathy for her current condition and the path she will have to go from now on.
But, the COLD hard truth is; I do not feel pity for anyone for disregard for their safety. They/she could have been mugged, raped, or even killed. The bottom line; she would not of heard those attackers coming at her either.
