Rio was in hot and humid conditions, so there was no incentive to go from way out, but it was also the logical culmination of a long series of time trial championship races in which there was no truly dominant, front-runner (post-El G), and no one wanted to be the sacrificial lamb. So, you got plenty of 3:35 and slower races. Centro with his tremendous innate racing sense and his dad and Salazar telling him to take the lead but go no faster than he had to, since he had a slower pr than numerous guys in the field, was just not going to go significantly faster than 4:30 mile pace until he had to. So, it was won in 3:50, with a 50s last lap but it could have been won in 4:00 if other guys hadn't started pushing with two laps to go.
I read they were cooped up in the call room for a TON of time. So nobody felt fresh at the start.
