I wonder also if you have an over corrected probation
If so cut down your orthotic
Last thing could be torque rather than impact stress
If the toe is fractured you will likely not help it by running through
If on the edge consider taping 5 and 4 together
Outside chance you have capsulitis
Few more ideas but these are the most probable

I'll bite. How about you go to professionals that deal with this daily instead of a forum with a mix of good, bad, and trolling advice (where it may be hard to differentiate which is which). Get a second podiatrist opinion, maybe more imaging, try a physio/PT. I'll speak to the last option. I've seen several instances of lingering pain after a stress reaction/fracture, or otherwise unknown lateral foot pain associated with the cuboid. Not saying that's your issue, but a sports or Ortho PT specialist could help differentiate (among other possibilities)
