This link has what is believed to be a copy of Landis' dope form after stage 17 of this year's tour. It is fromteh LNDD lab in Paris. The analysis and comments are very insightful as well.
I would start at the main page as their are more comments about this form.
The link for the actual forms is here.

Bottom line, the form has three columns for three riders. It lists the riders by numbers. The results for several different dope test are shown. The results are hand written and not the best of re-production quality.
Working backwards, it is beleived the riders on this form are Landis (who won that stage and is required to be tested), Oscar Perrireo (who was in yellow after that stage and is required to be tested, recall he finished second overall and will be named champion if Landis' appeal fails) and a third randomly tested rider.
If this is actual form, it says Landis' t/e ratio was originally 4.9 and then was msyeriously changed on another part of the form to 11.4.
The other (random) rider had a t/e ratio of 10. However, no other AAFs (adverse analyitcal findings) were reported.
Also, the rider beleived to be Perriero had morphine in his system, but was just below the WADA cutoffs.
