I just go by time. 30 minutes on a bike (or a little more) at a steady pace is like a 30 minute easy run. Since cycling is lower impact you could probably do a little bit more. Let's say you normally run 30 miles per week at 8 minute pace, I'd say if you cycled 35-45 minutes per day it'd be about the same amount of work. Sometimes do a fartlek and cycle extra hard for a couple minutes and then a minute easy for a workout.
1 hour is 1 hour dude.
I think getting your heart rate up to normal running heart rate is the best equivalent. It's much harder to do on a bike, but running is intrinsically more taxing.
Your heart doesn't know the difference between running and any other aerobic activity as long as you can replicate your running HR and activaty duration on a bike then it's all the same.. but feel free to take advantage of the no impact and build a bigger aerobic engine by going longer then you normally would
I agree with the previous points that 45min is 45min. I have a spin bike that measures watts, speed and Heart Rate. The most reliable is watts, then HR.
So say I run 8:00 per mile with AVE HR 130 for 1hr, then an about the same effort for bike/spin would be ave AHR 130 which for me is about 200-210 ave watts.
I find a can get the same aerobic work on the bike this way. Although my max HR is about 5-10 beats higher running. I've done some goal sets such as 3x10:00 with ave hr 155+ (250watt ave) on bike as a replacement for 2x10:00 (6:00 per mile) with ave hr 160+.
Not sure about concept II.

You'll most likely need some wire cables, 2" x 4"'s and some pulleys but it can most likely be done with a trip to Home Depot.
