I think a top level marathoner would be able to hit 45 miles in 4 hours. We'll never see it because what top level athlete would waste their time training for a 4 hour race in their prime?

There are lots of 50-mile ultramarathons run, and while most of them are on difficult terrain, surely some (perhaps small) fraction are on basically level roads with good footing. What are the winning times in the most competitive, fastest 50-mile races? (I don't know; I'm not an aficionado of ultras).
Easily. Don Ritchie was at 60.4 miles at the six hour mark during his 100k track world record in 1978.
