I can think of Indiana State, Wisconsin and Wake Forest. Please add to the list.

I know you mentioned Indiana State, but IU does as well. Pretty nice one too if you ask me.
Vassar College
Wisconsin's is not on campus...actually I don't even think it is in Madison
The team buses/vans out to it all the time to practice on but you can't run to it from campus (I would say it is about 10 miles one way)
That being said...very nice course. If they can figure out logistics (parking, etc...) they definitely should host nationals on this course some day
Lehigh has a great course that is with their athletic fields and facilities.
There are quite a few smaller (D2, NAIA, D3 schools) that have courses, but most people wouldn't know unless they have run there.

Virginia Tech's cross country course is right on campus. The start is about 200 meters west of the outdoor track.

What do you define as an "on campus" course? Indiana State's XC course is well over 5 miles east of campus. The course and recreation facility stands completely apart from the ISU campus.
Someone else said that IU has an on-campus course. Technically, that is probably true because it is on the IU golf course, but it is not connected to the IU campus in the same manner as the football stadium or basketball arena.

It sort of depends as what you define as "on campus." Lehigh and Lafayette, for example,have course at their athletic facilities which are located away from their main campuses but the courses are on their property They aren't using a park or golf course or some such thing. Some others that either have on campus courses or did when I ran in college forty plus years ago are (or were)
Colgate
SUNY Albany
Messiah (last I heard they are still using the on campus course they had when I raced there)
Pitt-Johnstown (really nice course, possibly my favorite)
Wisconsin-Parkside
Mt. St Mary's, Md. (from ages ago)
St. Francis, Pa. (ditto)

Rim Rock is a permanent XC course but is a 15 min drive north of campus.

Any major research university is going to have their recreational facilities separated from their "main" campuses, possibly by a good distance. So they may have a dedicated cross-country courses "on campus," but it doesn't mean you can roll out of your dorm room bed and walk to the starting line. University of Iowa is a good example of this-- it's a cross-country only facility, but it's a couple of miles west of the main campus.
Some smaller schools have courses that combine on-campus athletic fields, trails, and adjacent natural lands to make a course that is literally "on campus." Check out St. Olaf and Carleton.
How does this affect Mike Rossi and his efforts to clear his tarnished reputation?
You can add Cal Poly San Luis Obispo to the top of the list. There are about 10 million trail offshoots on campus. Search Poly canyon and you will see what I am talking about. SLO and the surrounding areas are probably the best places to train year round, in the US if not the world.
