Example:
- while I am over 30 with the BMI, my body fat % is single digit
- most runners are around 20 but they're SKINNY FAT and unhealthy
Why should this be rewarded?

Hey buddy, your body fat is not single digit if your bmi is over 30. BMI was created by experts in the fields of medicine and health. What are your advanced degrees again?
Right.... Here's the truth, I don't know why your physician is not telling you this:
BMI over 30 is physically impossible without AT LEAST 25% body fat. Dude, just admit it you are gross. You need to stop doing your pretend exercises and do cardio. Every day. Not only must you feel terribly ashamed of yourself when you look in the mirror, its going to kill you. Your horrible lifestyle and the extra weight you carry is going to cut DECADES from your life.
Insurance companies are run by people who understand statistics.

But for example someone like Klitschko, he is easily over BMI 30 but has hardly any body fat.

The marginal health benefits for going from 32 to 28 on BMI are greater than going from 28 to 24. Despite being a very lean distance runner, my BMI does not indicate an advantage relative to someone with a BMI four points higher. In fact, on a statistical basis, it probably has a longer expected life span; although those stats are complicated by people with low BMIs because they have a problem (smoking, already sick, frail, etc.).

Talking about BMI here, not % body fat. Most runners are around 19-24 for BMI.

If a company has a lot of employees, than offering a desirable incentive to employees for being under the "obese" threshold should lower their healthcare costs. Just because the number doesn't work in some special cases is irrelevant. As long as it's useful most of the time, it's going to work.
The thing is, these SKINNY FAT guys get a refund while I, with superior physique and health am left out. That is simply not fair and I am going to complain about it to HR.

I don't think most NFL running backs have a BMI over 30 like somebody mentioned. For example, I watched the Chefs/Bills game yesterday so I just decided to look up a couple stats. According to ESPN's player pages:
Jamaal Charles, 5'11", 199 lbs, BMI 27.8
Fred Jackson, 6'1", 216 lbs, BMI 28.5.
And these guys are the 0.0000001% exceptions. Sorry, if your BMI is 30+, you're fat.
