Idk where you live but that's absolutely not true. They estimate the taxes but at the end of the year when you file taxes you pay tax on actual tips. Not imaginary ones.
I ate at Denny's in CA (near LAX) recently, and tipped extra because the waiter gave me a plastic straw with my milkshake w/o asking.
Almost always around 20%. Mostly because the math is easy and I can always know I tipped "enough" -- no guilt whatever happens. If you had bad service or want to "get back" at somebody, you can always complain, leave a bad review, or better yet, don't go back. Don't withhold their earnings.
Double the first digit. If the second digit is close to 5, round up.
A more interesting question I think is how much you tip in a coffee shop, and what is the difference for drip coffee, espresso, etc. I almost always get regular drip coffee and I leave a dollar as my standard tip, because I'm often hanging out in there for a while (yeah I'm one of those laptop guys).
At all restaurants I usually tip 20% and round up to the next nearest dollar. So, if it's a $21 bill, I tip $5.
That being said, breakfasts can be extraordinarily cheap. If my bill is under $10, I still usually tip a minimum of $3. I actually tip that much for carry out and $5 for delivery.
