Usually they have a little hand or little foot. You usually can't see it.

"Lil" is a smart piece of reverse psychology which pays off in the bedroom.

"Dr" is used to secure good parking in the front of hotels.

Lil often implies that they are a child rapper or were a child rapper. Others probably adopted it from neighborhood nick names where they were the smaller of two people in the area with the same name.

Dr as far as I know is just used by dr dre and I think it was originally dr j in honor of his favorite basketball player then he changed it to dre from his first name Andre.

Great item in the book "Freakonomics" about the names given to black children. One guy named a kid "Winner" and another kid "Loser". "Winner" was a career criminal, while "Loser" had a very successful life. Still reading the chapter, but the conclusion so far seems to be that it isn't the name that holds back the black kids, but having parents that would name their child something stupid.

Freakonomics is pop-pseudo-science crap. Fun fact: people named lawrence are disproportionately likely to become lawyers; likewise for people named dennis and dentists.
