My coach had a three strike rule. Three missed practices = cut from team. I think though that if you are a top runner that he would be more lenient. I was a crappy walk on that would get cut without any sweat though.
I missed practice twice as a freshman. Once I overslept due to a late night out (totally my fault) and the second time was because my knee swelled up after biking all day Saturday. Unfortunately, the 2nd time, nobody believed my story.
The result was basically being ostracized by the upperclassmen. The coach never said a thing. (Actually, I'm not sure I was on his radar as a freshman. I was a walk-on and he didn't even know my name.)
Back in my competitive days I had a fairly strict coach. He would take away our cell phone if we missed practice. It sucked the big one, my man, truly sucked the big one.
In high school..worry about me, call my parents, send out a search team. I'm exaggerating, but the only time I remember missing practice was because I got in a car accident on the drive there. I was a good kid. As for the "bad" kids, which was probably only one guy, they couldn't run in the meet.
I just noticed that this said "Division I athletes," and I gave a high school experience. In college, I don't recall "missing" practice. I spent a lot of time in the training room and cross training (while injured), but my coach knew where I was.
Maybe you were at a D1 high school, so you were okay?
We didn't have to show up for the easy runs if we couldn't make it because of a conflict, but it was frowned upon.
I don't recall anyone missing workouts or long runs. Sometimes kids came in late and sometimes they got an earful other times nothing.
The coach would ask where the hell you were. Depending on your response and how good you were, the coach would then do anything from kicking you off the team to issuing a warning never to do it again. Division I athletics is very serious, so people would inexplicably miss practice almost never.

I cannot imagine missing 3 practices as a D1 athlete. That is like just missing three days of work. How does that happen?

Exactly. You're either paid to be there or you're a walk-on who has to be serious if you want to make/stay on the team. There's no good reason to skip (mandatory) practice other than emergencies, etc.
I played basketball in college and missing practice wasn't even a consideration. Class wasn't an excuse. We registered early to make sure there wouldn't be any conflicts. Missing high school basketball practice never happened, either, for anyone, including team managers.
One kid on our team missed practice once. The punishment? Castration.
my school didn't have an official policy, if the coach liked you most of the time you were ok. in my case he did not like me and so i was not ok...
