There actually sixteen spots. The top two individuals from each region who are not on qualifying teams attend the national meet. The exception to this rule is any individual on a non qualifying team who finishes in the top five of their region is taken to the national meet.
This year sixteen men qualified and the individual from Wheeling Jesuit did not finish. Most of the elite runners this year were on qualifying teams.

So do the top 5 automatically make it? What if the top 5 individuals are all from non-qualifying schools?
If you're in the top 5 at regionals, you make it to nationals no matter what.
