Very few JOBS pay in that range. most people making that kind of money are business owners. Careers that do eventual pay salaries in that range if your sucessful include: investment banking, private equity, venture capital, management consulting, corporate/securities law, and medicine. Pre-requisits for such careers generally include many years of education from top schools with top grades, decent looks, and reasonably strong sales ability.
There are many ways to make this kind of money as a small business owner or commissioned sales person, but those are obviously much riskier.
