That’s a crime
I have removed this risk from my life by never having eaten at a McDonalds in the 21st Century.
30 years ago I worked at a Sonic drive in and this guy I worked with would always do stuff to people's food if they were rude on the mic system. He would drop their hamburger patty on the floor or cough up a bug loogie snot and put it in people's cherry limeade. Looking back now I should have spoken up, but he was a grade older than me and would have beaten me up. He also got me the job there. I can honestly say I never harmed anyone's meal.

Years ago I was at McDonalds and when I went to order the girl went "SHHH!" at me and did the thing where you put your finger to your lips and she kind of glared. I was really shocked and I spoke to another member of staff about it. The next time I went there the same girl served me but she didn't speak. The next day I had food poisoning and I was really sick. I know it could be a coincidence but I can't help thinking she might have done something to my order. Maybe she got into trouble (not my intention). IDK.
When I worked as a waitress people did stuff to the food.
If you are a white person/family going thru the drive thru at most any fast food chain your food is definitely contaminated with bodily fluids of some kind. If I were one of you I'd definitely avoid. I figured this was common knowledge.
I worked at a number of restaurants, fast food joints, pizza shops, etc while putting my way through college. I've seen bad things.
I worked as a short order cook at Denny's when I was in college. Never saw or heard of waitresses or cooks doing anything like that to food. The worst I ever saw was someone sending food back for some reason, the waitress would ask for it to be re-made, the cook would say "sure" put it down on the counter for 5 minutes then nuke it 30 or 40 seconds and back out it went
The worst I saw at my brief stint at Olive Garden was someone sending back his iced tea to have a lemon put in it. The waitress was so put off by this that she went to the server station, said, "You want a lemon in your tea? HERE is your lemon!" and stuck the lemon in the drink along with her thumb. I thought about following the trashy waitress to the table and saying something but didn't (I was younger, I should have). I try not to think about it.
There was also a divorced woman who worked there who was super kind (she used to give rides to this guy without a car, etc.). She was a domestic abuse survivor. Some guys wrongly thought that she told the manager something that got them fired. They keyed he entire car (and I mean entire, it looked like the whole thing was scribbled on with scratches). There was also a guy who was addicted to oxycontin and another addict there. For many reasons, I don't recommend it as a job.

I was told to never get 🍋 lemon. You don't know if the person cutting them has hepatitis.
