He takes performance enhancing drugs (anabolic steroids). Not that anything he's done is particularly impressive but his PED use makes me dismissive of anything he does

Not saying he is clean, but he did compete in a "Natural" bodybuilding competition. Many people speculate that he's taking PED which may be the case but other than his physical appearance and lack of injuries with lifting and running across multiple years, there is no evidence.
I guarantee you that many runners could run 2:45 at 190 lbs. This is not that unusual. By that I mean it is much more rare to be able to run sub 2:10 at any weight.
As a weightlifter myself, his steroid use is not exactly a smoking gun just looking at that thumbnail. I know a few guys that are 100% clean that look like that but it comes with YEARS of heavy lifting.
Also how would taking roids help with his marathon time anyways
He competes in steroid-allowed bodybuilding shows and people who knew him from back in his days in the military have claimed he was roiding even back then
The steroids primary benefit isn't helping his marathon, but allowing him to maintain muscle mass while training for a marathon
Helping him maintain muscle mass is the one negative side effect, in his specific situation, of using steroids. It is a misconception (I think promoted by those with an agenda) to think that steroids only benefit sprint events. Steroids can absolutely help a marathon runner achieve faster marathon times, by, among other things: greatly reducing the time needed to recover from workouts, increasing muscle strength, tendon strength, associated decrease in injury risk, decreasing body fat, increasing red blood cell production, etc. Unfortunately we can't do human trials on elite athletes to prove this, however, any distance runner on HRT (test) will be able to tell you with 100% certainty that it helps, a lot.
No I know anabolic steroids help in the marathon, but that is not the main reason he takes them. Testosterone (and similar compounds) signal to the body to maintain metabolically expensive muscle. Anabolic steroids allow you to maintain a muscular physique while doing endurance training much more easily. The reason Nick Bare would take roids isn't so much to run fast (that helps) its to build his brand as a "hybrid athlete". He makes a lot of money selling junk supplements with this schtick.
