I’ve been a fairly active runner for a while now - now in university. In February 2022 my Achilles started playing up and I had an ultrasound, and these were the notes:
FINDINGS: At the midsubstance of the right Achilles' tendon there is fusiform thickening (8mm) with marked loss of fibrillar pattern, cystic changes and interstitial partial thickness tears. Moderate neovascularity is present. Appearances are suggestive of advanced tendinosis. No full thickness tear seen. Cortical irregularity is also present along the surface of the calcaneum leading to some minor insertional tendinopathy but no tears or calcifications seen. No retrocalcaneal bursitis. Normal appearances to the proximal tendon and myotendinous junction.
Impression:
Advanced tendinosis with interstitial partial thickness tears to the midsubstance of the Achilles tendon. No full thickness tear.
I’ve tried:
Heavy weighted calf raises (single leg and double leg, straight knee and bent knee)
Taking vitamin C and marine collagen before calf raises
Taking time off - several months and then slowly progressively loading back
Heel cups
Custom orthotics/insoles
3x shockwave therapy
Ibuprofen/voltarol gel
Achilles massage
Changing shoes
I managed to get running again for a few months between November 2023 and April 2023, but the pain has pretty much always been there in the background or flaring up. Driving for long periods and going on the elliptical for extended periods can also be quite painful. Any advice? What do I try next?
