Does everyone at US nationals get drug tested or just the people who finish top 3 and then a couple of Randoms? I am curious of a couple of lower tier runners who I know and suspected they could have been using something to qualify and wondering if this was validation that they were clean or if they just don't all get tested at US nationals.

Not passing a test at a major event is more of an iq test than anything. The time to catch people is unannounced and intermittently (including “off-season”).

A very large segment of the finalists get tested.
They will all know this and as others have eluded, the smart ones will make sure that they are not glowing.
Regular blood work can be used to make sure that you can ride the line without going over even on out-of-competition tests. Obviously, some exogenous compounds if discovered in even trace amounts will still result in a fail.
So not everyone that qualifies gets tested. In my mind I always assumed you were tested before the meet and after the meet. I thought the same with nationals until I qualified
