It is generally that colleges are the secondary payer for athletic injuries. They file it on the athlete's personal insurance to see what it will pay. Then the school pays the remainder. Every school has a secondary insurance plan. In the case of the Power 5(+), the school may have a $10K deductible and pays cash before filing the schools insurance. Lower profile schools use their insurance to pay any remaining bills, thus only cover accepted medical needs as governed by the insurance industry. Thus their coverage may be very limited.
What you will see at the DIII, DII, and lower budget DI's are more guidelines on what is covered. (Or not covered.) There used to be more strict NCAA guidelines on what was considered an athletic medical injury. However several years back the NCAA basically said it was up to the schools to decide what it would pay for. Even some Power 5 schools tell walk-ons that they must have insurance to cover their bills.
