This can only be explained by a sad lack of aerobic fitness. Immediately begin working towards improving her lactate threshold. 2 mile runs at about 18:06 pace per mile should do the job. For her next attempt, enlist a rabbit to hit precise marks of 3:58, 4:01 (7:59 at halfway), 4:02 (12:01) and then it's about heart. Remind her as she barrels down the final 100m at a blistering negative splitting pace that an ice cream cone at the corner store awaits for a magical sub-16:00 time.
As for the WR, a 3 year old boy ran 25:00 for 5K. I'd say your girl is, at the moment, out of luck. Keep training, add some hill repeats, and she can put records out of reach.
