Is there an equivalence chart/formula for sprinting on a treadmill (speed versus incline)? If I want to run a 50 second 400 (~18mph), but the treadmill only goes to 12mph, what incline would be used to equate this?

Yes, there are such calculators, but it is completely different mechanically to run at a slower pace up a hill, so there is no equivalence. The 'equivalence' is more for aerobic training, i.e. metabolically (not mechanically) running up a hill for a while is similar to running more quickly on the flat. In particular, a 400m sprint does not fit into that.
Here is an example of a decent calculator.
Should use a non-motorized treadmill for sprints.
True Form Runner, Assault AirRunner or other alternatives.
