1. If you look a little further down on the same page you're quoting from, sirpoc writes about the benefits of setting 70% as a cap. After he wrote the book, he seems to be even more convinced that it really should be a cap (for example on the FOD podcast).
2. For me at least, weekly sessions of 10x3 + 5x6 + 3x10 has always meant 90 minutes of work. Not including the rests.
