Yeah, it’s pretty bad. Based on his Truth Social posts, he seems to have gone around the bend. So far, other than his rallys, he has made any public appearances and it’s fortunate for him that he doesn’t need to attend debates.

If they act like fools, it just gives that little bit of ammo to DJT, so that he has a grain of truth at the core of his mountain of lies when he attacks the media. There is a difference between smearing people, giving them terrible nicknames, and being otherwise vicious and what happened here. Those things, as distasteful as they are, could all be considered strategic and intentional.
Confusing Pelosi with Haley and going on and on with the mix up? That's cognitive decline.
Republicans can nominate him anyway, and I'm sure they will. But when they come for Biden for a gaff or two, they are going to get both barrels blown right back at them with endless video of Trump saying things like this (or thinking that Obama is still the President, for example).
I think the stress of all the court cases is getting to him and he's starting to crack.
Trump HOPES the economy crashes this year. He wants to be president, and he thinks the president can have people murdered and legally get away with it.
