The five year mortality of treated / untreated "aggressive" prostate cancer (pre metastatic) disease is pretty similar. It's hard to know how effective our screening programs and interventions are in the very old, as there is a lot of competing risk at that age (people die of other things).

Biden's annual medical reports did not include his PSA scores. There is no way it was not known that he had prostate cancer before he was elected over four years ago. One of treatments is what is basically chemical castration to stop the production of testosterone. This causes low energy, depression, and brain fog. All of these symptoms were observable before he was elected.
By the way Trump also doesn't disclose PSA scores.
You can have relatively high PSA without prostate cancer and vice versa. You need PSA, plus imaging plus biopsy if imaging (MRI) gives a high Pyrad score, e.g. Pyrad 4 or 5. If all those are "high" next step is biopsy where multiple samples are taken in the prostate. Only then do urologists have a clear picture (but still some subjectivity because those who read images give different scores....so it's not a perfect system) and can make recommendations. At this stage the Gleason score is important. Gleason 9, which Biden has, indicates an aggressive prostate cancer.
For the record, yours truly has Gleason 6, and is under "active surveillance" with PSA tests every few months.
You are absolutely correct. One of my clubmates has PSA numbers in the mid teens, yet he has no prostate cancer. He's had 4 biopsies and all have come back negative. I have a PSA score of 5.0 yet I'm also under 'active surveillance' for two cancerous cores from my biopsy. A Gleason 6 (grade 2) and a Gleason 5 (grade 1). Hope active surveillance was the right call. My next biopsy is in June.

As a fellow stable genius, I also thought that perhaps nuking a hurricane off shore would eliminate said hurricane… but after my PR team told me it was ridiculous and the very idea of it made me look like an idiot I retroactively decided it was a joke.
Retroactively changing the seriousness of our ideas and claims is the hallmark of us genius’s. Think about guys like L Ron Hubbard and Joseph Smith, who both have reputations as stable geniuses and not any other shared reputations.
I don't like the man's politics but I respect him and he is a fighter. He was an appalling president who was too far gone to recognise his own 'decline'. However Gleason 9 plus bones isn't necessarily instant death (ianad). This gives him hopefully some time to continue his work against cancer and to concrete a positive legacy for himself despite his presidency.

A colleague of mine from Saudi teaching has Gleason 9, and metastatic, since 2 years ago. He's living normally ....with hormone therapy. That zapped his libido, btw.

Good luck Sir. We are kindred spirits, living with P C. Gleason 6. I would rather avoid prostatectomy and also different forms of treatment and I would absolutely prefer to avoid hormone therapy.
My PSA just under 10 is not moving, since diagnosis, February 2024, so the urology team has given me the option to continue active surveillance but of course that may change the future. I have to live with this beast but I'm finding out it's very common and we're in good company.
I'm fortunate to live in the United Kingdom where treatment is absolutely free with no costs including for all the procedures. If you have prostate cancer in the United Kingdom the National Health Service acts super fast and gets the treatment done, if necessary. Nursing teams are empathetic.
