The medication is taken by a lot of people for other reasons so I don't believe that it's a case where it's a death sentence if he is being given the med. If he's taking it, I'm sure he's getting hourly bp readings. The problem is that the doctor did not say Trump is taking it nor would be be taking it as it is generally used and not as a preventative.

There is no study (even a bad study) that shows hydroxychloroquine to be effective in preventing COVID-19 infection. That's what Tiny is using if for.

The doctor is in a tough position. Usually, your patient is not also your boss. So if Tiny wants a drug it is hard for the doc to say "no".
Medically, I think the doctor is in the clear though. Doctors are allowed to prescribe drugs for "off label" use like this. And aside from ICU patients, Tiny is probably the most closely monitored patient in the world. So any sign of adverse effects will be dealt with quickly.
Lastly, it's my understanding that doctors often prescribe a drug just because the patient wants it. Docs know that the placebo effect is very strong so if the patient believes it will make them feel better then it probably will.
That's part of the reason why drug commercials should be banned again. Another reason is because drug companies now spend several times more on sales and marketing than they do on research and development.

This is the reason I have my doubts about what Trump is saying and what the doctor's letter says. If Trump was taking the medication, you'd expect a doctor to write a little more specifically that he's either been prescribed the medication, it was recommended to him or that he's taking it. Plus I think a doctor would add, he's taking it while being very carefully monitored by the WH medical staff. Since the letter did not say any of that, the entirety of the composition is suspect. The letter was written by someone who is not a medical professional.
Rather than finding a solution to the problem they simply try to hide the data for personal gain at the expanse of we the people who they are paid to represent. About half the population will excuse it because it’s “their side” and they’ll continue to get screwed.
