Okay, there are a lot of varying degrees of issues being described here. If you are running for years with pain at a marathon level, and you are just now going to a PT AND and few sessions help. You were simply ignoring a minor problem that could've been alleviated years ago due to being stubborn. that is not major haglund's/tendonosis issues.
Now if you've tried everything with PT except for injections, tenex, surgery. Your running career is not over but you have a persistant/recalcitrant problem. You need to trust your medical professionals and get real treatment.
Like a few poster's have mentioned, getting the surgery will most likely result in pain free activity.
Here's a list of current pro's that came back from haglund's issue's to run kinda sorta ok (if you consider Olympics, Olympic trials, world championships as ok):

Like the NFL, NBA, MLB and other professional sports teams, Olympic runners are in a different situation than most LRC people! I would never recommend surgery for running injuries because in most cases there is a practical cure. It will not get you running a 3:59 mile, but it will get you running.
