He said a month ago that his symptoms came on 1-2 days after his second Pfizer jab. I don't know his detailed health info. but given his history and the timing of this extremely rare embolism condition, I would think the vaccine is the number one most-likely cause. I hope this has been reported to VAERS by his health care provider(s) since it is required by law.

That does NOT sound like Covid at all. I wonder if you have something else going on, such as an underlying infection like Bartonella or Lyme. Bartonella causes nerve pain, dizziness, trigeminal neuralgia, and both Lyme and Bartonella cause migraines and trouble swallowing. These are bacteria that can lie dormant in the body until an infection weakens your immune system -- like the flu. Bartonella can also cause sinusitis.
If you continue to have lingering health problems and tons of mysterious symptoms down the line you should consider seeing a LLMD for proper testing. A regular doctor will just tell you it's "chronic fatigue," or 10 years from now tell you that you have MS.

I know a guy who smashed his calf on a boulder on a climb. It got really bruised but didn't think any of thing of it. A couple of weeks later, he was hacking and really sick. A trip to the ER found that the trauma caused a clot that moved to his lungs.
Made me wonder if Sage took a tumble on the trail in the past month or so.

1-Sage's NUMEROUS healthcare providers were/are required by federal law to report his condition to VAERS. Failure to report is actually a felony (fwiw) and filing a false report is also a felony (as expected). From the Pfizer-BioNTech's own Healthcare provider fact sheet:
"The vaccination provider is responsible for mandatory reporting of the
following to the Vaccine Adverse Event Reporting System (VAERS):
...serious adverse events* (irrespective of attribution to vaccination)....
... Serious adverse events are defined as:
• Death;

I've been able to return to some running this week (granted on flat bike paths in Boulder)...just taking it easy at high 7-min pace and keeping the HR in the 130s.
For perspective a month ago I absolutely could not hold 9-10min miles without having to stop and walk. Going up a flight of stairs walking would leave me breathless. It felt like my heart and lungs were going to explode and I was on top of a 20,000' mountain.
I had no major bumps or injuries this year. I've had some mild tendonitis and inflammation around my hip/glute/si joint for months (I think) and wasn't running as much this winter/spring though. I also did a few high altitude uphill ski efforts pretty hard this spring and it's not like I wasn't totally inactive.
Other than that I was pretty careful about COVID exposure and even though I took a few short plane flights between CO and CA about 2-5 days before feeling very bad lung/chest pain and pressure symptoms, I never felt sick or anything. No fever etc. Whether or not I was exposed and infected with real COVID this Spring I can't tell right now because the antibody test I took a few weeks ago likely tripped positive from the vaccine (I thought it could differentiate antibodies between the real thing and the vaccine but apparently not).
And yes, I did get the Pfizer vaccine around that time as well. My initial symptoms (feeling like the intercostal muscles around my lungs were tightening up) occurred about 30 Hours after my second dose. A few nights were so painful that I couldn't even lie down to sleep and so I slept sitting upright on the couch. Of course later that week I was misdiagnosed with "Pneumonia" (apparently) and suffered from chest pain and not being able to breathe very well for nearly a month after that before this PE diagnosis. Since the doctors told me I had Pneumonia I really thought I had Pneumonia, but now in hindsight some of my symptoms (coughing up small streaks of blood in my mucus) and the fact that I was so out of breath even after 4 weeks makes more sense now. Ultimately it was my lack of ability to run and fairly intense chest pain around my heart that prompted me to go straight to the ER.
I have personally reported my case on VAERS in elaborate detail including the lot number on the vaccines as well as my full emergency room report (CT scan with contrast was what caught the PE) my heart was under a decent amount of stress as well and the initial EKG was a bit concerning. Heart ultrasound has shown the heart is normal and looking pretty good now though and an EKG later returned to normal. It felt like a weight has been lifted off my chest.
I am determined to try to find a cause to this all. The doctors don't know and more testing needs to be done. I am consulting several specialists. A big key factor (besides the various cancer testing I've already started) is the possibility of some sort of genetic pre-disposition to blood clotting factors. Apparently quite a few people have stuff like "Factor V Leiden" etc. They can't test me for that now (or for about half a dozen more genetic clotting diseases) when I am still on blood thinners though.
I don't have anyone in my close family history that I know of with any of these issues though.
So until those tests are done (and it is determined I'm cancer free) it is hard to say what might have caused this. There are rare genetic structural things as well like "May-Thurner syndrome", but again more testing needs to be done. I'm trying to convince my doctors to do some more ultrasounds, but they generally have been talking me out of it....at least for now.
Despite being very pro-vaccine and pro- science generally I'm not going to ignore the timing of when I got my COVID vaccine and the onset of my symptoms. I feel like that would be irresponsible.....especially considering there have been a few hundred cases of myocarditis/ pericarditis from mRNA vaccines now. The odds that happening seem to be quite rare though...like in the realm of 1 out of 100,000 or higher? However, apparently it is yet to be seen that blood clots from Pfizer (especially DVT to Lungs) have been reported all that much? Statistically people get PEs all the time and if that happens to be the week they got a COVID vaccine we can't necessarily prove that was a cause or influence at all. Like maybe I just "had it coming," More likely real COVID can cause blood clots still it appears (and note that I didn't get J & J or Astra zeneca.....)..but all this should be really rare. So I'm left asking the questions: "Why Me?". "What are the odds?". I'll admit the timing is very interesting though. Until more testing is done I don't think anybody can point to a likely cause at this time though.
Disclaimer: this is all my opinion as of now (And it may change over time). Obviously I'm not a medical expert. I'm just trying to report the facts as I've experienced them so far.

Bret Weinstein (eminent biologist) last week had a roundtable with the developer of the mRNA technology and a microbiology entrepeneur that went deeply into vaccine side effects, including obvious manipulation of the VAERS system by the "powers that be". It was called "How to save the world in three easy steps and was three hours long. Coagulopatholgy was a concern discussed. Unfortunately, but predictably, YouTube removed it.

Going through something similar but not as bad as you, after second shot of moderna, though I had covid last year. First shot no problems.
Self-treating with Natto and Ginkgo to break up clots and thin blood because no insurance unfortunately.
Feeling bit better after 30 days, hope you continue to progress, take it slow, it's the summer no rush.
My girlfriend (who is in her 30's) developed fever, chest pains, and difficult breathing after her second Pfizer vaccine dose. She was first diagnosed with pneumonia then eventually lung cancer. I don't believe the vaccine causes lung cancer.
