The FACT is that health insurance premium subsidies are ending. Even if you want to lie about these subsidies going to illegal immigrants there is no denying that they also go to tens of millions of Americans. There is also no denying that once you take away these subsidies, health care costs will increase massively for the people who receive them (who are pretty much uniformly poor). Every credible health policy org says this will happen.

But if NY (or CA, etc) goes to the Feds for reimbursement, or shuffles the $ around so as to in effect get the Feds to pay, that is is the issue. On the "hard working" US citizens, I think one of the issues is some people who are not really in need become eligible for Fed assistance in some states. Like most Fed programs, there are many opportunities for abuse or cheating.
That's not what Vance said. He accused New York of spending federal money on US citizens, and spending state money on "illegals".
What Vance is saying is simply not a problem for the federal government, and the Republican solution is to take away the federal money for the US citizens, requiring the state to fund federal Medicaid with state Medicaid funds, for those states that provide Medicaid.

There are solutions to ensure that federal reimbursement is limited to what is intended and permitted and not fraudulent. And Democrats were and are still willing to negotiate to address any federal funding concerns for "illegals", but it is the Republicans who have refused to open up any negotiations, using fabricated allegations of fungible spending, if not fraud, as a pretext to kill healthcare nationwide, while blaming the Democrats and the undocumented migrants, and give the funds to the rich.

Kicking the same people off healthcare that were without healthcare under Obama, and basically every other administration prior to Biden. COVID era expansions were never intended to be permanent. Most of the people who are uninsured will remain 18-39 year old working aged adults who choose to go without. Kids and the elderly are almost universally covered regardless of whoever sits in the White House.
