I don't like the man's politics but I respect him and he is a fighter. He was an appalling president who was too far gone to recognise his own 'decline'. However Gleason 9 plus bones isn't necessarily instant death (ianad). This gives him hopefully some time to continue his work against cancer and to concrete a positive legacy for himself despite his presidency.

A colleague of mine from Saudi teaching has Gleason 9, and metastatic, since 2 years ago. He's living normally ....with hormone therapy. That zapped his libido, btw.
The idea that this has anything to do with anything but being 82 is absurd. Prostate cancer diagnoses at this age are extremely common. They say that this is a type amenable to hormone treatments, so perhaps he can be treated, though it looks bad at this point, having spread to the bone.
No, it's true. Something like 80% of men over 80 have prostate cancer. If you autopsied every guy who died in his late 90s and did a thorough prostate biopsy, you'd probably find malignant cells in just about every one of them.
Not really relevant to this discussion, though. The common prostate cancers are indolent and would take decades to become clinically significant. Obviously not the case for Biden's cancer.

The news articles all try to be upbeat, but the reality is that agressive prostate cancer is often fatal, especially for seniors with pre-existing health conditions.
They knew about this for years. Over the age of 50 people with good MDs are getting PSA tests done annually. 5 year survival rate is almost 100% with state I, II and III. For it to have gotten to stage IV is very rare.
I would wager he knew about it and didn't want surgery.

The most medically tested man in the world was never diagnosed with stage 1,2,3 cancer. The teams of medical professionals didn't read the monthly blood test results of an 80 year old man for 4 years while he was President. They just happened to notice AFTER he was president.
