Look into how many children under the age of 5 die every year in Africa from largely preventable diseases and conditions. While using the woods as a toilet or having to dig a daily cat hole seems like a major first world problem, there are plenty more significant places to throw your money towards.
Lack of sanitation and waste management is a major source of those preventable diseases. So I would say a toilet can be very significant. India has a guy called "Mr. Toilet" trying to modernize sanitation because it's such a huge public health problem.

Let me tell you something about Kenyans. - I’m online friends with approximately equal amounts of Kenyans and Ethiopians (all have a running interest or are runners of varying abilities) whilst absolutely none of the Ethiopians have ever asked me for anything, the same cannot be said for the Kenyans. Literally all of the Kenyans have at some point asked me for differing amounts money.
In my experience Kenyans are all about the money. They do not help each other financially (I have a friend who trains with some of the best pro’s but although living in poverty they do nothing to help him financially) Ethiopians are a more independent and proud people in my experience. Kenyans can be very underhanded making up devious and untrue scenarios in order to elicit donations from others.
My experience is why I’m pretty sure many Kenyans dope in order to win prize money. This is not something I attribute to Ethiopians however……
I read or saw somewhere that Ethiopian runners are more likely to be from the middle class. I'm not sure why. It sounded like they need some safety net to take up pro running, like being supported by their family or savings. Not anyone just gives it a go, like in Kenya where it sounds like they have nothing to lose.

many of the largely preventable diseases and conditions can be caused or exacerbated by poor sanitary conditions
I remember walking into an elementary school in Iraq many years ago during the war. The "toilets" consisted of a hole in the ground. Basically, it was a non-portable Port-a-John. The urinal was simply a long trough along the wall, much like you see in large sports stadiums. Except instead of the urine going down a drain and into a sewer system, the trough was slightly angled toward the wall. There was a brick missing at the end, allowing the urine to flow outside. Outside I was greeted with a huge puddle of disgusting smelling urine that was covered with flies and other insects.
We don't understand how good we have it in the US.

ie sh1tting in the same water where clothes are washed, bathing is done, and dishes are rinsed.
It was the same with Bett's family. They couldn't even afford to give him a funeral.
All the freaks here who get off on poverty stricken East Africans being doped to death by Italian and Spanish crooks, just so you can wax lyrical about 'genetically thin calves' or 'bi-pedal hunters born for running half-a-million years ago', and yet none of you would send $5 to help their families out.
