FWIW 19 hours / day moving is not an atypical strategy for 6-day runners. Some do more. For 10 days, yes, it would be tougher. I’d bet John Geesler has done it with less rest.

There is no official recognition for Trans cons. Everybody runs on different roads. It's more a mythical challenge. You can never prove that you have done a Trans con. But some runners are way more believable that they have done it on their own than others.
In this case, your buddy Nedd challenges the world ultrarunning legend Yiannis Kouros, who ran 1000 miles under controlled and certified conditions. Your buddy ignored all that, invented that genius "Master lap" thing, hired a timing company and a track. It's a big show but it's not good running practices.
But hey, he is doing it "for the homeless". We have heard this many times. Usually it's "for the kids".
Absolutely, the back half of these races can be brutal. Injuries, accumulated fatigue, sleep deprivation, digestive problems....it all in piles up.
This is closest Nedd has become to measuring himself directly against other runners and his inexperience in teaching him a harsh lesson about just how adrift far he is from the very best.
Another point of comparison, we have the Sri Chinmoy 3100 Mile currently underway. Ashprihanal Aalto's race record of 40 days 9 hours. That's running 1000 miles in 13 days three times in a row, plus another day and a bit for good measure! Not to mention the runners are only permitted to race a maximum of 18 hours each day.
If Nedd finishes in 13 days, I don't think he'll be too keen to turn around and repeat the feat twice more.

I'm cynical, obviously, but I looked into this, I'm confident the charity, and his role is very much above board.

He looks like he's running really well to me. Covered a lap in lane 5 in 2:20, so that's under 9 min mile pace. The lads on the podcast just admitted the record is gone, but he's still pushing for the 1000 miles. Fair bloody play.

How is this any different from the NYRR taking advantage of crippled people for the wheelchair/handcycle races in a marathon so they get suckers to donate to the NYRR and then pay their NYRR CEO an obscene salary of US$600k for what is supposed to be a non-profit?
