I don't disagree with you, either enforce the rule or do away with it. You and I are 100% in agreement there. That said, it is a current rule on the books (both IAAF and USATF) so an athlete who is in violation could be subject to a penalty. I'm not aware of any sports where rules aren't always judged 100% by the book at all times. Basketball, sometimes refs will call contact as a foul, sometimes the exact same contact will not be called. Football, there is holding on pretty much every play at the college and pro level, the refs choose to call it only a certain % of the time. Not saying that is right or wrong, just saying that as an athlete you need to understand the rules of the sport and if you get called for an infraction that actually occurred that isn't anyone else's fault.
As far as elites handing bottles to each other, technically because this is an individual sport they are not handing bottles to a teammate they are handing bottles to a competitor. The IAAF still doesn't allow it (pg 249 of the 2014-2015 rule book) but I've never seen a DQ based on someone offering a bottle to a competitor. In the spirit of competition I think this would be seen as a sporting gesture.
As far as an athlete taking a bottle from a race official on a bicycle, IAAF rules permit this (pg 249) if it is "under the direction of race officials". Seems unfair to me for race officials to be able to help certain athletes in this manner and not others, but I don't really know what those who made the rules were thinking. To me it's much more sporting if you have some random person on the side of the course offering water or an orange slice to any runner who comes by than for a race to specifically assist some runners by biking drinks over to them.
