Obviously it worked for that guy in Boston - you still remember it 36 years later!
Two reasons I believe he did it.
1. He didn't want to get interviewed with the media and all that other attention when he had to go to the bathroom so badly.
2. It was used to get him a better contract at a better race, as 2:07 stopping to take a piss is more impressive than 2:06 without.

The classic would have been if he had to drop trou for 5 min and came blasting out of the Sanikan to sprint by Arciniaga FTW.
The winner of the Rome marathon in 2010, an Ethiopian, stopped in the last mile to *take off* his shoes for the finish. Race organizers were offering a bonus to the winner if he did this. It was supposed to be a tribute to Abebe Bikila. I believe he ran around 2:08:30.
