Tips start at $5, as far as I am concerned. I delivered pizzas for a while, and I can still see the face of the jerkwad that tipped me ten cents.
But yes, the delivery fees are shady, but don't take it out on the delivery guy. All the delivery guys I knew were trying to save up to do something else. Or, they were putting themselves through school. Give them a hand.

I had a friend in college who delivered. Money was always tight so he was happy when he drove up to a nice house and delivered. It all came to $16.67 and they guy handed him $20 and told him to "keep the change". Cool, a $3 tip.
He said thanks and walked back to the car. He got a knock on his window and looked up and it was the guy he just delievered to. The guy said "Gimme the $3. I said keep the change." He emant the 32 cents. HA!

I am in a urban location so many of the deliveries are from stores within a few blocks walking distance.
I typically tip 5 bucks but I fit the middle aged white guy stereotype. At the very least tip $2, it's not the law, you can tip zero and have all the excuses in the world but it's a karma thing. Be a dick, get dick back.
$2 to $5. If it's some monster order for a party or something you would obviously tip more.
I delivered pizzas for a Domino's in a college town long ago. The money was really good (for a dirt bag job) but we were so busy that it was insanely stressful at times. We were being sent out with between 2 and 6 orders at a time. Shifts started at 5pm and we got out of there at about 2:30am. I don't think I ever walked out of there without more than $100 in tips and gas money. Usually it was a lot more. Throw on top of that minimum wage and it was a good job for a college runner during the summers.
Cell phones must be so amazing when delivering pizza. Holy shit I could have used one about 8 times a night back when I delivered.
I ran all my second runs at 3am for two summers in college. It was a trip. Loved it in a weird way though.

Why do places charge a delivery fee?
To deliver, a place has to either provide their own cars and hire drivers (not very common) or have the drivers use their own car and pay them a fee for that.
They have two choices of how to cover this cost. They can either charge a delivery fee or add the cost into the price of the pizza. The problem with adding it into the price of the pizza is that it makes that place a lot more expensive than the place that just has pick-up service. Some places do have pick up only specials where they sell the pizza at a cheaper price.
The cost to have delivery service is about as much as the cost of the ingredients in the pizza. Anyplace that tried to absorb that cost without charging the customer would soon go out of business.
The above is a myth ... drivers got the "per-run" gas money (mine is right now $1.10 per run) before the delivery charge, back when delivery was still "free"
Companies added the delivery charge but the amount the driver got did not change.
The delivery charge is to add profit to the company's bottom line ... no more, no less.
To clear up a few things/restate:
Delivery charge is just so the company makes money. Avg tip at least working for round table in a suburban area is $3+. I used to bus tables and worked way harder to earn about the same amount of money in tips. What I have learned is that people are way more critical toward servers than most other professions for some reason. I think server's at places where bills come out fairly low should be tipped better they work as hard or harder than those at fancy places. In conclusion, any delivery driver who gets a $3 tip and complains is ridiculous, $2 is fine but low, $4 or better is great.

I stopped tipping during the recession.
When is this country going to abandon the practice of paying people extra money just for doing their job? The cashiers at Zankou Chicken have tip jars now, Baskin-Robbins too. It's a strange phenomenon. The "service industry" line of reasoning is weird too, because everyone in the workforce is selling their services - that's what it is to be working.
That said, I tip the waitresses at my favorite restaurant whatever the nearest dollar over 20% is, because they know how I like my food prepared.
