BK is the best of those three. Whopper with cheese is the best "standard" burger from any of those places.
Of course In N Out is better by a mile than any of those, so is Five Guys (though they need to toast their buns), and so is Whataburger (which I had for the first time in summer 2010). Jack-in-the-Box is also better, and so is Steak and Shake. I don't like Carl's Jr. though.
For those of you in the Columbus, OH area, the three-location Grafitti Burger serves up the best fast-food type burgers in the city if you ask me. I've not yet had a Thurman Burger at Thurman's Cafe, but I don't consider that "fast food".

In N Out is regional, and Five Guys really isn't the same category- a burger and fries there costs closer to $8 than $2.

In the spirit of this thread, you make good points. Wendy's, BK, and McDonald's are really the big three when talking about hitting every region of the country.
I prefer McDonalds.
I am looking forward to trying the new Wendy fries, though.
Wendy's only because they now have decent salads. Burgers and fries from any of the three are complete crap.
