If somehow a test was introduced that caught any cheaters immediately, so nobody doped. How different would athletics look within a couple of years? Would it be unrecognizable? The Japanese dominating the marathon etc?

I think we can get a clean sport using a dipping version of an implant or GCM. If you have a device taking readings continuously even a micro dose would be detected.
It would be intrusive, but it might be a solution to a problem like Kenya. Give them an ultimatum - accept that all of your runners have to have this implant, or accept a global ban from athletics.
