The risk is high that it's wishfull thinking from your side.
The way to make your move is to meet her outside of a pt session in a normal environment and then see if she is still interested.
For example ask to get a coffee at Starbucks or invite her to a Party.
If she comes there and is still flirting with you where right otherwise it was wishfull thinking.

OP, do your PT and yourself a favor by sparing the poor woman from your inappropriate sexual advances. She's not into you, and she's going to rebuff you.

Almost half said they’ve experienced one of these situations in the past year - numbers that haven’t changed since the last major surveys in the 1990s.
Researchers found that 84 percent of survey participants had experienced inappropriate patient sexual behavior during their career, and 47 percent experienced it during the last year.
Women [working as PTs] reported significantly higher rates of harassment, especially staring, suggestive remarks, inappropriate touches, date requests, sexual gestures, requests for sexual activity and masturbation.
Harassment was most common between a female therapist and male patient. Treating mostly male patients increased the odds of harassment by almost 400 percent, and treating an equal mix of patients doubled the odds, as compared to those who mainly treated female patients.
My guess is the arm issue relates to biceps tendonitis and gooning for hours on end to pawnhub. Lots of repetitive movements with a slight bent arm is going to give you biceps issues. It’s highly likely the PT knows this too and is probably trying to suppress laughing out loud when you describe your symptoms.

I noticed when I walked into the exam/treatment room, she left the door wide open. And before testing the strength in my arms, she put on gloves. We seemed to have a good rapport, and she only hurt me a little. Fast forward to this morning: this time she almost closed the door all the way. It remained open a small crack. I’m not sure if that was intentional or not. This time, when she tested my arm strength, no gloves! We discussed a couple of hobbies we have in common. How do I make my move? I’ve been to physical therapy before where massage was part of the treatment. How do I guide us down this avenue?

OP is a creep seriously, wanting to wait for her to finish up her day to ask her out. Dude think at all about how that is normal? The women has a doctorate which means she is educated. Women of her caliber are not interested in some creepy moron that is going to wait for her to get off work. What you explained your plan is sounds like the start of a missing women case.
