Any deaths in a sporting event is tragic. Comrades is unique in the size of the field , the cut times, and the fact it takes place on a paved road. Because of how long you are out there when running an ultra, there are bad things which can happen to your body. For example, if you lose 1 liter/hour of water during a 4 hour marathon you could be miserable but will probably stagger home salt caked and hobbling. If you misjudge water during a 12 hour ultra (either too much or too little) the implications can be much more severe and in a field of 15,000 runners it is probably more difficult to identify runners showing heat stroke symptoms
The one race which I have been surprised we have not heard more about was the Gansu 100K ultra that took place in China in May of 2021. There were 21 fatalities out of a field of 172 runners due to an extreme weather event.
