If you aren't entirely comfortable with the offer from company A (and you don't hear from company B by 11/26, or whatever the job offer acceptance date is), then consider staying at your current job as a "known entity" versus making a commitment to a new job with a new company that you have some reservations about. You could say that you have some personal/family issues that conflict with the travel at this time (or something similar). They have other candidates they can contact, and you have other jobs/employers to consider, but it will just take more time.
As far as being blacklisted goes, that generally happens when you get offered a promotion with your current employer, and it appears that you don't want to take on the added responsibility. (In an extreme case, some companies have an "up or out" promotion track.) But for changing jobs to a different employer, you can contact them in a year and say that you're now free to make a change when you weren't before.
This seems like great advice and info. Thanks for this, even if I'm not the one who needs it.

I have not yet received a response from Company B. On Friday at 6pm I emailed Company B and advised them that I have another offer which requires a response by 11/26.
This afternoon I called Company B to see if they received my email. No response yet.
If I don't receive a response from Company B by tomorrow afternoon, I think I have my answer...

I'm 50. Been there. Done that. Uber elite status with airlines and hotel chains, etc but no life at all. It's miserable. I made a good deal of money and it wasn't worth it. 30% pay rise? Sorry, I dont know what your making but traveling 100% has a HUGE cost on your life. If you're not making millions it isn't worth it. And I didnt pull millions from my ass, BTW. Literally meant millions. dont piss your life away.
Do not do this, especially being a 30 something male - you are in the prime of your life.
Last night was very stressful.
I've been battling with whether or not I should accept the offer from Company A. I simply don't think I can do 100% travel. It's such a drastic lifestyle change that would be very difficult to manage.
I still have not heard from Company B. I'm hopeful that I have a decision from them today. Especially since I advised them that I have another offer which requires a response by 11/26.
Option C is going back to the drawing board and sending out more applications. The thought of staying at my current employer for another 3-6 months is very troubling.
I work in healthcare and we recently had a survey completed by the joint commission. The surveyors were onsite for four days doing all sorts of things to accredit us. I got to know one of the surveyors well and while chatting about how much travel he does, he said he does it full time and enjoys it. He (and the other surveyors) had a median age of 55-60. Essentially they travel from Sunday to Thursday or Monday to Friday, and their area's are the entire U.S. So no regional only travelling. On that note he also said that his kids were out of the house, he was working towards retirement, and said younger professionals started out as only half time because of family obligations or other professional obligations. Once you got older, it was easier to go full time because there was less obligation at home. He also said the travel was not all that it was cracked up to be as they rarely had time to really see the places they went to. It was mostly airport->car rental station->inside of hospital->restaurant-> hotel room and repeat.
Anyway, good luck with whatever decision you make.
I've declined the offer with Company A. It was one of the most difficult and stressful decisions I've ever made.

Getting pretty anxious about why I haven't heard back yet from Company B.
On Friday I sent an email to the hiring manager asking if it was possible to have a response by 11/26.
On Monday afternoon I called the hiring manager (and recruiter), and left a voice message asking if they received my email.
I haven't heard from them since the interview on 11/18, one wee ago...
Just got an email from Company B. They are moving forward with another candidate.
I am devastated...
There is no way you should take this job. Goes against your life plan and against things that make you happy. I left newspaper reporting for similar reasons (all events I had to cover were at 7:30 at night. No thanks. Not good for a family man or even a married man.
We're only on the planet so long, and there are things beyond our control that make us miserable sometimes. No sense in choosing something that will make you miserable.
You did the right thing. You would have been miserable with Company A.
Most of the best decisions i've ever made in my life i didn't know were the right ones until months or years later.
So you stay put for a while. It's not the worst thing and i am pretty confident that the offer that really fits what you want will come along.

These big decisions are seldom "black and white" issues, and you are often dealing with incomplete information. From your posts, it sounds like you had some reservations, and you passed. Be confident in your decision and that there will be other opportunities. One of the pieces of wisdom in the book "101 Things I Learned in Business School" is: There's a Trolley/Streetcar (= new opportunity) every 15 minutes.
