Internal testes, because there's no other way natural testosterone could get to those levels.
Also, artificial testosterone would be distinguishable from natural so it would show up if doping was implicated.

I have no issue with Caster Semenya being treated as a woman because that is how she was born and I am guess that by outward appearances is a woman. Women have been competing this way through the history of sports (she is not the first, nor will she be the last), so it is not some new standard from a historical perspective. I'm wondering if all of the holier than thous who are saying she should compete in the men's division would feel the same way about someone born with a penis but internal ovaries would feel about them competing in the women's division.
