I would have to say the scene in Misery where Annie hobbles Paul's ankles. Also I was watching Braveheart recently and although they dont show it he got his penis and balls cut off among other things.

Anything from Taken. Most notably the scene where he confronts the Albanian kidnappers or where he has a “friendly” dinner over at Jean Claudes: “it’s a flesh wound!!!”
Not the most intense scene, perhaps, but the final 5 minutes of fight club are pretty damn intense.
