For reasons that are about to become apparent, I'm not gonna write this post using my regular posting name, but I have A LOT more experience with this name situation than I should, and I can't recall anytime it's gone horribly wrong...I tend to forget people's names immediately after I meet them, especially when I'm drinking (kind of ironic since I'm usually quite good at memorizing things).
There have been a fair number of times when I'm talking/dancing/hanging out/making out with a girl (even going home with) whose name I have totally blanked on. Obviously, you found yourself in a similar situation and are now contemplating further relations w/ said female. I've learned a few tricks that may be of help:
1. Checking the driver's license...if you go somewhere to drink, she'll have to take it out. Think of some reason to make a comment, which can be easy if she has a unique/out-of-state license. Maybe laugh and be like "haha, that looks like a fake I used to have, here lemme see that." Then tell some story about how you altered an ID w/ a felt-tip marker to make your first fake (true for me), and how scared you were when you first tried it (also true). Make sure to look hard at the back as well, so it deflects suspicion.
2. The phone...last spring I woke up with a young lady next to me, and her phone was also on the bed. I could barely remember the night before, much less where I ran into her, and so as she was passed out I took a quick look at her phone, and one of the last messages from her friend had the girl's name in it (it turned out I had met this girl the previous fall so I actually had her number in my phone, which I confirmed by calling my number on her phone - you probably won't be so lucky, so think of an excuse to check out her phone "wow, that's a cool phone, can I see it?" which works especially well with the new smartphones since there's tons of places the owner's name could show up).
3. Facebook. Just say "add me, here's my email," or "what's your email, lemme add you," or think of some excuse to send her an email like you wanna forward something to her...then send a follow-up with some question or whatever that requires her to reply.
The language thing would also work if you have some tact...but think of reason to get on the topic first.
Places to avoid are places where you may have to write down her name: bowling alleys, bars where she may wanna play darts/pool, or where you could run into friends, especially if they say, "Hey who's your friend?"

I noticed this thread yesterday and read more today, and I guess it speaks volumes to the level of excitement in my life when I'm finding myself overly interested in whether or not this dude got her name or not and how he managed to do so.
Maybe she knows you don't know her name, and she's going to purposely withhold her name just to see how long she can go until you actually admit it and ask her.
