Blacked-out in the middle of a hot humid 10k run down roads lined by cornfields in Midwest Ohio. I did finish. Regained my senses with a bowl of puke in front of me and I was blathering on about some nonsensical shiit. The race director was cool though and found the key to the local pool which was next door and let me jump in to cool off.
Maybe not the worst but:
1. Stopped to eat some blackberries on a wild bush by a running trail and fell prostrate into the bush. Was heck getting out. Thorns stuck in my arms, the side of my face, and scratches all over my bare chest and stomach. I yelled for help but no one around so had to get out myself.
2. Found a little girl by the side of the road on the grass in a park. She had blood coming out of her nose and was not conscious (I thought maybe dead). Fortunately she was only sleeping. She had gotten separated from siblings and had got a bloody nose and cried her self to sleep. I searched the park with her tell I found her siblings.
3. Was trying to show off to a girl friend senior year of high school by hurdling a shopping cart on a run. Made it, except for the tip of my trailing toe, which sent me into the dirt which I'm sure impressed her lots. Especially when I was picking rocks out of my body.
4. Had a big rock barely miss me coming from a fenced residential area.
5. Had a funny feeling on a night run and turned around to see a very large white german shepard coming at me not making any noise with fangs bared. Probably 20 ft away. Glad I turned around and was able to get him to leave.
6. Peed myself at the end of a quick interval while just wearing running shorts around other people while in college.
7. Had to poop in a hedge in someones driveway off of a fairly busy road. Was afraid of someone coming out of the house really pissed.
8. Had some married lady proposition me while in college while running at night around the track (I didn't know she was married until she told me while I was talking to her while jogging). I said I needed to get going and got out of there.

I was out road running during college and was listening to my discman and there was a stray dog on the opposite side of the street. I could tell he was off leash and a girl was trying to get him to stay. The dig and I locked eyes and he went into the street to approach me and BAM! Smacked by a car.
Then there's the time I sprained my ankle the only one time I decided to run while stoned.
heat exhaustion about 10 feet from the finish line at Bloomsday. Core body temp was 108°. Got packed in ice, went hypothermic (down to 91°) and had to be heated back up. Felt like I had been hit by a train after that.
