This is the correct answer. It's very easy to do, especially if she has to pull it out when entering a club, purchasing alcohol, or ordering drinks at a restaurant.

Make a funny comment about your own I.D. photo, then show it to her. If she already has hers out, tell her you want to take a look and compliment her on how much better she looks. She will just see it as small talk.

Or, introduce her to a friend, and say "this is my buddy I've been telling you about, (friend's name)", then just kind of pause, and let her fill in the rest when she realizes your friend still hasn't heard hers.

The ID check might work but has its dangers. If you stare at it intently enough to make sure you're reading the name right, she may notice. Also, when you didn't pick up her name at first meeting, what if she originally identified herself to you by a variation of her name (or a middle name that she uses socially), rather than the "official" version of her first name? She'll notice the difference and know you're going by the ID.

Anyone been on the other end of something like this? I had introduced myself to this girl a few weeks before I asked her out. On our first date, I had the feeling she didn't know my name or had forgotten it. I made some comment that one of my friends told me "Mr. Oblivious, you are the biggest dumbshit I know" .... or something to that effect which worked my name into the conversation. I ended up dating her for a few years. She said later she knew my name but I think she's lying.

ask her if she has any nicknames or what she likes being called, then even if you don't get her real name out of that you'll be able to call her something

or tell her to add you on facebook because you don't go on it that often or something like that

Youc could get her to drive you somewhere. Encourage her to speed (where it's safe). Then listen closely when the police take her name.

Sing Eminem's 'Real Slim Shady'. Repeat the 'Hi my name is....' bit quite a lot. Maybe she'll join in and add her name.

Carve your name on a tree in the woods. Carve a big heart. Then tell her she has to finish it off so you both contributed.

Challenge her to a round of golf. Sign-off her card at the end of the round.

Well done, except for #1, which is a really bad idea. I also like the idea of calling when you know she doesn't have her phone (not from your # obviously) and getting the name from the message. Finally, if you're at her place, she must have a piece of mail/magazine/etc you can get her name from (maybe when she goes to the next room to riffle through your wallet and figure out your name)?
