I have run under Jerry Mizner before, but I think that's not very creative. Some teammates ran as Kronen Bear and Phineas Q. Butterfat. A little more creative.

I was working a race and gave my buddy (#4 finisher) a free entry, so without him knowing, I signed him up for a fake name. He had a girlfriend named Mary Kate and everyone used to make Full House jokes around him.
Raplh Wiggum is one I've seen quite a bit. I like to use Scandinavian names that the race directors can't pronounce at all, like Kjell Svensdertroem or Bjorn Kjolen Hjomelsonn, etc. From now on I think I will use Brandon Moen, though.
Olden Phatte
A friend of mine thought it would be a good idea to race under the name Myha Balzich. The race director even read it out as he crossed the line.

Ralph Wiggum is one of my favorites, also. I think I remember a poster on LetsRun once pointed out a Ralph Wiggum from a race and wrote "Me finish 5th? That's unpossible!"
UMW.
For some reason we ran a road race during our cross season because there were no decent cross races that weekend. We decided to have a little fun.
After reading through these, I think I am going to only use fake names from now on in road races.
Does anyone remember when Mark Champion won the 10k at the Charlotte Observer Marathon?
Well, a pretty good boxer just beat Adam Goucher by one place at World XC Championships.
He goes by the name of Mohamed Ali.
